Set up HSEQ

Choose the disciplines and categories your organisation uses, set the consequence ladder, and set numbering, investigations, permits, the risk matrix and who is notified.

Assignar turns HSEQ on for your organisation. Open Settings and select the HSEQ card. You need Configure HSEQ. Without it you see You do not have permission to configure HSEQ.

Select Save when you finish a block. You see HSEQ settings saved. If it fails you see Could not save HSEQ settings.

Let a raise rule decide from the answer

At the top of the card, switch on A raise rule can use the answer to choose positive or negative. It saves as soon as you switch it. If it fails you see Could not save. Try again.

A form's HSEQ raise rule can then use Decide from the answer as well as Positive and Negative. See Field types.

Choose disciplines and issue types

Under Disciplines and issue types, each discipline is Quality, Safety, Environment, Community or Security. Tick the issue types that discipline uses: Observation, NCR, Punch List Item and Incident. Quality has no Incident tick. Safety also offers Personnel non-conformance.

A discipline with no ticked type shows Disabled — no issue types are enabled for this discipline. Open the discipline to manage its categories.

Record breaches by a worker

Personnel non-conformance is off until you tick it under Safety. The Consequence ladder then appears below the disciplines.

The ladder starts as Chat, Verbal warning, Written warning, First and final, Second written warning, Final warning and Terminated. Edit a step's name in place. Move up and Move down reorder it, and Remove deletes it. Type a name in the empty box and select Add step to add one. Select Save.

A record keeps the consequence name it was given, so changing the ladder does not rewrite existing records.

The first time someone with View HSEQ personnel non-conformances opens Raise an issue, ten Golden Rule classifications are added under Safety, such as Not wearing required PPE and Working at height without fall protection. Edit or deactivate them like any other category.

Only owners and admins hold View HSEQ personnel non-conformances. An owner or admin on an access preset needs it ticked in that preset. Raising and managing the records is in Record a personnel non-conformance.

Add a category

Inside a discipline, select Add category. Enter the Category name. Choose the Raise form crews complete when they raise that category, or leave No form.

Evidence required makes a completed action attach evidence before it can be completed. Review required sends a completed action for approval. Active keeps the category available on new issues.

Named location required lists each issue type against each discipline. Tick a cell when that combination must name a location from the project tree. A point on the map does not replace that location.

Decide who may close their own work

Completer may also approve lets the person who completed an action approve it, or verify its NCR. Never in the Safety discipline keeps that rule off for Safety even when the switch is on. When the rule blocks someone, they see The person who completed the work cannot approve or verify it.

Set response windows and numbering

Response windows sets the default hours for each priority and the due-date offset for each severity. Empty means no default. Changes apply to new records only. A value that is not a positive whole number shows Enter a positive whole number, or leave the field empty.

Numbering pattern sets how new issues are numbered. The preview appears when the pattern is valid. If a number is already used you see That number is already in use. Check the numbering settings, or contact support if it keeps happening.

Create a record PDF

Create a record PDF when an issue reaches and Create a record PDF when an action reaches choose the statuses that file a PDF. Upload print template replaces the layout. Active template is the layout used for the next PDF.

Choose the frequency-rate denominator

Frequency rates is Per 1,000,000 hours (AU/NZ) or Per 200,000 hours (OSHA). The HSE statistics page uses this denominator.

Set up investigations

Investigations holds Methods, Layers, Factor types and When an investigation is required. Add a method with Add method, then its layers and factor types. Deactivate an item to stop offering it. Investigations that already use it keep it.

Rules run from top to bottom. The first active match wins. A rule compares the Incident's classification, outcome and severity. Any matches every value of that part. Add rule needs an active method. You see Investigation settings saved, or Could not save investigation settings.

Running an investigation on an Incident is in Investigate an incident.

Choose who is notified

Notifications sets who hears about raised and closed issues in each discipline, and when an overdue action escalates. Action assignments and reviews always go to the responsible person. Select Save settings. You see Notification settings saved, or Could not save notification settings.

A project can keep its own recipients. Create project override starts that copy. Reset to company defaults removes it.

Each person can mute raised and closed updates for themselves from My notifications in HSEQ. Assignments, reviews, rejections, due reminders and escalations still arrive.

Add permit types

Permit types is the catalogue used when someone requests a permit. Enter Name and Code, choose the Permit form and, when close-out is required, the Close-out form. Leave Active on, then Save. You see Permit type saved. A code that is not 1–6 letters or digits shows Use 1–6 letters or digits. Issuing a permit is in Issue a permit.

Set the incident risk matrix

Incident risk matrix is the likelihood by consequence grid that rates an Incident. Save matrix stores it. You see Risk matrix saved. Incidents already rated keep their rating until someone assesses them again. Reset to built-in restores the 5 by 5 matrix.

Add project locations

Locations for a project are on that project's Locations tab, not on this card. You need Configure HSEQ or Update projects. The steps are in Put issues on the site map.
