Raise and manage issues

Raise and close out an HSEQ issue, including a restricted personnel non-conformance.

Assignar turns HSEQ on for your organisation. You need View HSEQ issues to open the register and Raise HSEQ issues to raise one. Assigning, sharing and closing someone else's issue needs Manage HSEQ issues. Passing or failing verification needs Verify HSEQ issues.

In HSEQ, select Issues.

Raise an issue

Select Raise an issue.

  1. Choose the Discipline and the Type: Observation, NCR, Punch List Item or Incident. Quality does not offer Incident. An Observation also asks Positive or negative.
  2. Enter the Title. Choose the Project and, when that type requires it, the Named location.
  3. Choose the Severity: Minor, Major or Critical. Set Observed at and the Due date, or leave the due date blank to take the account default.
  4. Turn on Assign to me when you are the owner.
  5. Answer the linked Field form when the category has one. When it does not, the form reads No Field form is linked for this category. You can still raise the issue.
  6. Select Raise.

You see Raised and the issue number. Open it from the register.

On a Field form, Raise issue on a line opens the same form with that line filled in. The type and location come from the line when the form supplies them.

A positive Observation is recorded Closed. It has no Add action control.

An Incident also asks for a Classification, Outcome (Actual or Potential), Raised by, and the client and regulator notification fields. You see Choose a classification for this Incident until the classification is set. The investigation is in Investigate an incident.

If the title is empty you see A title is required. If the category requires a named location and you have not chosen one, you see A named location is required. A due date before the observed day shows The due date cannot be before the day this was observed. Any other failure shows Could not raise the issue. Fix the field the message names and select Raise again. When the issue is raised but the form is not, you see The issue was raised, but the Field form did not save. Open the issue and complete the form there.

Record a personnel non-conformance

A personnel non-conformance records a breach by one worker. Your organisation turns the type on in Set up HSEQ. You need Raise HSEQ issues and View HSEQ personnel non-conformances. You cannot raise one from a Field form line.

  1. Select Raise an issue. Choose Safety and Personnel non-conformance.
  2. Enter the Title and choose the Project.
  3. Choose the Worker. The list holds your own workers and the subcontractor workers approved on that project, with their organisation's name.
  4. Choose the Classification, the rule that was broken.
  5. With Manage HSEQ issues, choose the Consequence, or leave None yet.
  6. Select Raise.

Prior records lists this worker's earlier personnel non-conformances and the consequence on each. It reads No earlier records for this worker when there are none.

You cannot change the worker or the project after raising. If the worker is not on the project you see That worker is not on this project. Without a worker you see Choose the worker this is about.

Who can open it

Only people with View HSEQ personnel non-conformances open the record, see it in the register, or are notified about it. Field users never see it. The owner and every action assignee must be a person who holds the permission, not an organisation or a tag.

Set the consequence and close it

The issue shows the Worker, the worker's Organization when they are a subcontractor's, the Consequence and Prior records. With Manage HSEQ issues and View HSEQ personnel non-conformances, change the Consequence while the record is open. It saves when you choose it. If it fails you see Could not save the consequence.

Close issue needs Manage HSEQ issues or Verify HSEQ issues. When every action is approved or cancelled, the record closes itself. While it is open, the raiser or the owner can withdraw it as Raised in error if they still hold View HSEQ personnel non-conformances. Withdrawal removes it from HSEQ totals.

Find an issue

Filter by project, location, asset, discipline, status, issue type, positive or negative, category, severity, owner and the dates it was raised. Select Apply. Clear filters shows every status, including closed items. Columns chooses, pins and orders the columns. Views saves that layout as a personal view.

The empty list reads No items match these filters.

Assign it and set the due date

Open the issue. Under Next step, Assign (or Change) opens Assign owner. Choose Person, Organization or Tag, then Save assignment. You see Assignment saved.

The due date is on the issue. A manager changes it from the issue details. You see Issue updated.

Move it on

Next step names what happens next and shows the button for it.

  • Start work moves an Observation, NCR or Punch List Item to In progress. On an Incident the same move is Start investigation, and the status becomes Under investigation.
  • Send to verification moves an NCR to Awaiting verification once its actions are finished. Pass verification and Fail verification need Verify HSEQ issues. A fail returns the NCR to In progress.
  • Close issue, or Close observation on an Observation, records the close. An NCR also needs a Disposition: Rework, Accept as-is or Reject. Accept as-is asks for a closing comment of at least 10 characters and a signature.
  • Withdraw issue asks for a Reason: Raised in error, Duplicate or Other. The person who raised it, and who does not have Manage HSEQ issues, uses Raised in error instead of the full withdraw dialog.
  • Escalate to NCR on a Punch List Item keeps the record and gives it an NCR number. This cannot be undone. You need Manage HSEQ issues.

While actions are still open, Close issue stays disabled and the page reads Close is unavailable while actions are still open. Closing then shows Close this issue after its actions are finished. An NCR closed without a disposition shows Close this NCR with a disposition.

Actions on the issue are in Work corrective actions. Sharing an NCR is in Share NCRs with a client.

If someone else saved the issue first, you see This row changed since you opened it. Reload to continue. Select Reload. Without permission to change it, you see You cannot change this issue. A closed issue shows This issue is closed and cannot be changed.
