Investigate an incident

Record an Incident, start its investigation, and submit it for approval before the Incident can close.

Assignar turns HSEQ on for your organisation. Raising an Incident needs Raise HSEQ issues. A restricted Incident opens for people with View HSEQ incidents, and for the people named on it: the raiser, the assignee, the investigation lead and team, and the action assignees. Who can see this? on the Incident lists that set.

You start the investigation record when you have Manage HSEQ issues, or when the Incident is assigned to you. You approve or return it with Hseq Investigations Approve.

Methods, layers, factor types and the rule that says an investigation is required are in Set up HSEQ.

Raise the Incident

On Issues, select Raise an issue. Choose a discipline other than Quality, set Type to Incident, and choose a Classification. Set Outcome to Actual or Potential, and record Work related, the client reference and when the client was notified.

Select Raise. Start investigation under Next step moves the Incident to Under investigation. That button changes the issue status. The investigation record is the panel below.

Start the investigation

On the Incident, the Investigation panel asks for Method, Lead and Due. The lead defaults to the assignee when you leave it blank. Select Start investigation. You see Investigation started.

When no method is active, the panel reads No active investigation methods are available. Add one under Settings → HSEQ → Investigations, then come back.

If the save fails you see Could not update the investigation. A withdrawn Incident shows A withdrawn Incident cannot be changed.

Record factors, a narrative and witnesses

Under Contributing factors, describe the factor, choose its Layer and Factor type, and select Save factor. A layer that requires an action needs a linked action or a Reason for no action. Link an action attaches an open action already on the Incident. Raise an action adds one. You see Each factor that requires an action needs a linked action or a reason for no action until that is done.

Add narrative opens the method's narrative form. Add witness statement attaches a witness form. Add person puts a Lead, Member or Reviewer on the team. Add comment is visible to the team and the approver.

Download report saves the investigation report.

Submit, approve or return

Select Submit investigation when the record is ready. You see Investigation submitted. The status becomes Submitted.

The approver selects Approve or Return. Return asks Why is this being returned? Leaving the note empty shows Add a note saying why the investigation is returned. The lead sees the note and the status returns to In progress. Approve shows Investigation approved.

The Incident cannot go to verification until the investigation is approved. Sending it earlier shows Approve the investigation before this Incident can be sent for verification. If someone else saved it first, you see This investigation changed since you opened it. Reload to continue. Without permission to edit it, you see You cannot change this investigation.

Closing the Incident after that follows Raise and manage issues. Rate it first with Save risk assessment on Risk assessment. Pick both a likelihood and a consequence, or neither. You see Risk assessment saved.
